1. If a form asks for a
generic NAME field, ALWAYS flow First Name
then Middle
Initial then Last Name then Suffix in
order. UNLESS…If form asks for "Name
shown on return", must
match the name exactly and be consistent!
IMPORTANT NOTE:
If the “Name shows on tax return” does not
clearly identify "suffix", in this case do not
