                           This form must be completed if you're licensing a location  in Wheat Ridge.
                                     Due Date:  20 days after opening the business
                                     Applicable City Code sections:  22-66(a) and 22-66(b)

       All supplies, equipment, furniture, fixtures and leasehold improvements used in business are
       subject to sales tax.  Credit is allowed for tax properly paid to another city (such as when you 
       pick something up at a store), but please be aware that if the other city's tax rate is less than
       3% you must pay the difference to Wheat Ridge.  A tutorial is available on the website or
       in print form on request.  Tax staff is also happy to answer any questions you may have.
