Purpose of This Form

Use the Wage List Adjustment Schedule (Form C-7) to correct Social Security number, name or total wages reported on or omitted from a previously filed wage report.

Who Can Use This Form

Employers and agents can use this form to correct previously reported total wages or to correct the Social Security number, name or wages of employees previously omitted or incorrectly reported.

Requirements

This form must be signed by an owner, officer, partner, or individual with written authorization to represent the business.

Submit a separate form for each quarter being corrected.

Other Options for Adjusting Reports

You can complete the Wage List Adjustment Schedule (Form C-7) online using Unemployment Tax Services. Adjusting reports online requires registration to Unemployment Tax Services. During the quick and simple registration process, users create a User ID and password to use each time they access the online system.
