Reporting Options
To ease the new-hire reporting process, an employer has a number of options available
to report employees' new hire information. You are encouraged to submit
New-hire/Re-hire Information on a CD or electronically. This
benefits both the Employer and the New Hire Registry in processing the information
accurately and efficiently.
PAPER- Employers may mail or fax
any one of the following:
- Utah New Hire Registry Reporting Form 6; Click

New Hire Reporting Form*. This form
may be copied as needed.
- Employee's W-4 (Employee's Withholding Allowance
Certificate)
- Computer Printouts, or other printed information
that provides all seven of the mandatory data elements.
Regardless of the form used, information should be typed or printed legibly.
ELECTRONIC MEDIUM - Employers may submit reports using:
TELEPHONE - Employers may call the Registry during normal business
hours of 8:00 a.m. to 5:00 p.m. Monday Friday MST to report up to three
individual new hires at one time.
Regardless of the format used, employers should make certain that all of the
required information is included. The employer street address should
be the address where child support orders should be sent. For employees with no
Social Security number, reports should be held by the employer until a number is
obtained. Incomplete or unreadable reports will be returned to the employer
and may be subject to penalty if not resubmitted properly.
