                How to Reinstate  

                        Domestic Limited Partnership (L.P. - including L.L.P.) (U.C.A. Section 48-2a-203.5) 
                                    Domestic Limited Liability Company (L.L.C.) (U.C.A. Section 48-2c-1208) 
                               Domestic Uniform Limited Cooperative Association (U.C.A. Section 16-16-1212) 

                        You may only reinstate within two, (2) years of becoming Involuntarily Dissolved.

*Appoint a new Registered Agent and Office with a Utah address and be sure this new Registered Agent signs as
indicated on the Application.

Change Form to the Division of Corporations. You may file in person, mail, fax                                          , orbyuploading the pdf form(s )
to our online Shopping Cart using a credit card for payment. Means of conventional payment are, cash, check, 
money order made payable to the "State of Utah". If you are faxing you must include, on a cover sheet, the 
number of a VISA, MasterCard or American Express with the expiration date & CVV #.
