Substitute Form Scenario Requirements

Submit three copies of each return with a completed Transmittal for Substitute Form Approval. Each copy of the return should include separate scenarios for each filing type (such as married, filing separate, or different checkboxes). Use fictitious names, ID numbers, and addresses.

Elements to Include in Return Copies [+]

The elements below apply to the following returns:

For individuals: Individual Income Tax and Property Tax Refund
For businesses: M2, M3, M4, and M8

Include one of each of the following scenarios in your submission:

Maximum field length data on all fields, all checkboxes marked, and only the checking account box marked in the banking section. Use more than Xs and 9s.
Negative values (where allowed), no checkboxes marked, and only the savings account box marked in the banking section.
Variable field data and various checkboxes marked.
