Refund Request Forms

Refund requests must be made within three years from the date of your tax payment, the date the tax return was due, or within three months after final determination of your federal income tax liability, whichever is later.
  • No refund will be issued for amounts of $10.00 or less
  • This refund may result in an amendment to Federal, State, or other City tax returns
  • Refunds over $10.01 are reported to the IRS
  • Please allow up to 90 days processing of your refund request
