Please file signed tax forms along with a complete copy of the Federal return, any required supporting documents/explanations, and any payment due by the appropriate State of Ohio due date. Returns may be submitted to City of Lebanon, Tax Dept., 50 South Broadway, Lebanon, OH 45036.
