and submit for filing along with: 
–     A $30 filing fee.
–     A separate, non-refundable    $15 service fee also must  be
      included,  ifyou drop off the completed form.
Note:   Upon the filing of this form by the California Secretary of State, 
the registration of the LLP in California will be cancelled.
