The DE 2063 is a form used by employers to certify their employees for partial benefits. Employers can print and complete the Internet version for submission to EDD. The Traditional Chinese version is a sample and is to be used as a guide when completing the English version. The EDD sends this form to a person’s last employer when a new claim is filed, or an existing claim is reopened. You may receive a DE 1101CZ or a DE 1101ER. The form will include information that is specific to the claimant. The last employer completes portions of the form and returns it to the EDD. (The forms on this webpage are samples only and should not be used to provide information to us.) Employers can also submit information in a letter. Include the claimant’s Social Security number and use the return address on the DE 1101CZ/ER, or DE 1545 for mailing. The last employer can electronically receive and respond to the EDD’s Notice of Unemployment Insurance Claim Filed (DE 1101CZ) using the State Information Data Exchange System (SIDES). Notice of Wages Used for Unemployment Insurance Claim
The DE 1545 is mailed to base period employers after the first payment has been made on a claim. Specific information regarding the claimant will be entered on the form. Base period employers complete portions of the form and return it to EDD. (The version on this website is a sample only and should not be used to provide information to EDD.) Employers can also submit information in a letter. Include the claimant’s Social Security Number and use the return address on the DE 1101CZ, or DE 1545 for mailing.

Notice of Required Retroactive Certification
Due a historically high number of unemployment claims, the EDD automatically processed Unemployment Insurance and Pandemic Unemployment Assistance benefits early in the pandemic through the week ending May 9, 2020. Claimants are now required to certify for those weeks known as retroactive certification. The UI Branch will be referencing this form in other forms once the retroactive certification period has ended.
