1D barcode instructions for software developers:
The 2 digit year code should reflect the tax year "19"; The Vendor Code
field should contain the software developer 4 digit assigned code for
identification purposes; The Form ID is assigned to each individual page
by the Department of Revenue. The name of the form page is listed next
to the Form ID code. Entries in the 2019 1D Barcode Layout page
highlighted in yellow are new additions. Entries with strike throughs are
removed for the current tax year.
A one dimensional barcode must be generated at the top of every page of
every individual income tax form and schedule.
Location on page: Printed on the upper-center portion of the return with
ΒΌ inch of surrounding white space.
Size and type: 30 point, FREE 3 of 9 barcode font
For the 2019 1D Barcode Layout, please be sure to use the 2019 Column
for form numbers. 2018 numbers are in the layout as reference.
Individual Income Tax Forms Only: Effective 7/15/2019, the Vendor
Code and Form ID within the 1D barcode for the Individual Income
Tax forms is being switched. The value found in the 1D barcode
should also be provided as human readable text. This can be
accomplished by using the WASP tool. It will automatically format to
a Code 39 mc font slightly below the 1D barcode. Microsoft San Serif
and Calibri font are also acceptable. The human readable data
should be scaled to fit underneath the barcode and should not touch
the barcode data.
Courier/Courier New, bold font, size 11 or 12 are also acceptable. The
human readable text should be the same width as the barcode.
