Colorado Substitute Form Developer
Welcome to the Colorado Department of Revenue’s Substitute Form Developer home page!
Substitute Form Developers are required to receive approval from CDOR before the release or distribution of any replicated forms.
To get started:
Obtained a 4 digit vendor identification code (per product) from the National Association of Computerized Tax Processors (NACTP). Obtaining a vendor ID is free by filling out the Vendor ID Request Form.

Complete the current year Letter of Intent (DR 5029) (link found below).
Read the Substitute Form Developer Specifications and 2D Specifications (Found on the SFD Specifications page. Link below.)
Review submission guidelines (Found on the SFD Specifications page. Link below.)
Once the LOI has been approved, the substitute form developer may send replicated forms for review.
